Group Plan Design

OraQuest Dental Plans offers two group DHMO plans, the 110-01 and the 120-01.
These two plan options may be offered on a stand-alone basis, alongside each other
as a high/low option or in conjunction with our indemnity option (dual choice).

Individual Plan Design

The OraQuest 120-01 plan also serves as our individual option and may be sold by
monthly bank draft or annually by check/credit card.

Dual Choice Plan Design

The OraQuest Dual Choice plan combines the DHMO plan with our indemnity
component and this option has minimum participation requirements. The indemnity
plan allows the member to select any dentist and incorporates annual benefit
maximums, lifetime deductibles and waiting periods for major services. Additional
information for underwriting purposes may be required for this option.

Group Enrollment Application

OraQuest Dental Plans requires the following on new case submissions:
- Application for Group Dental Service
- Group Dental Service Agreement
- Employee Enrollment Form
- Check for first months premium

Individual Enrollment Application

OraQuest Dental Plans requires the completion of the Premier 120-01 Enrollment Form for the individual plan.
